Mental Health Community Support Services
One-on-one support for independent living.
Our Mental Health Community Support Services (MHCSS) program provides one‑on‑one assistance to help adults 21 and older live with greater independence and confidence. From education on health and medication management to guidance with budgeting, nutrition, and housing stability, we walk alongside you in building practical skills for everyday life. We also help you access community resources and foster meaningful relationships that support long‑term wellness. With Medicaid, DAP funding, and private pay options, our goal is to make personalized support accessible to those who need it most.
Psychosocial Rehabilitation
Build confidence, life skills, and community.
Our Psychosocial Rehabilitation (PSR) program offers a supportive, group‑based environment where adults can build essential life skills, boost self-esteem, and strengthen social connections. Through engaging activities focused on health, safety, budgeting, and community involvement, we help participants develop tools for greater independence. The program also creates opportunities to form friendships and give back through meaningful volunteer experiences. Open to adults 18 and older with a qualifying mental health diagnosis, PSR is available through Medicaid, DAP funding, or private pay, making it an accessible path to growth and connection.
Assertive Community Treatment

Assertive Community Treatment (ACT) is a team-based mental health service where patients with severe, persistent mental illnesses, like schizophrenia and bipolar disorder, receive a wholistic treatment plan from our medical, behavioral, and psychiatric care providers. ACT is designed for people who need more than traditional outpatient care—especially those who struggle to stay connected to services or whose symptoms make everyday life difficult to navigate. Our team of professionals can help bring personalized care directly to these patients, whether that’s in their communities, their homes, or where they feel most comfortable.
